Thank you. It must have been a horror for you as it is for patients. The system is awful and the people are hoping to hang on to their jobs. Who cares what goes into the record? An RN today asked a patient who requested a comprehensive report if she'd like her to tell the MD what to put into an amended report. Patient was aghast. She wanted a comprehensive eval not just more "don't sue me" boilerplate. Again, thanks.
